We are looking for motivated, enthusiastic individuals with appropriate experience and registrations to join a dynamic multi-disciplinary team within prison healthcare as a Primary Care Counsellor.
CHCP CIC currently provides integrated offender healthcare services across HMP Hull and HMP Humber. We deliver a community- evidenced based model, bringing clinical expertise to the patients in our care, working closely with the multi-agency workforce.
The post holder will be expected to work as a member of the recovery health team which incorporates both mental health and substance misuse. This is within a secure environment, under the supervision of the management structure.
Main duties of the job
The post-holder will be expected to undertake patient assessments, plan and advise on referrals to determine appropriateness to undertake counselling.
They will be responsible for managing and prioritising their own workload and caseload, to meet the needs of patients. This includes working within the Integrated Health Services, facilitating a range of individual based care planning, group work and 1:1 evidence-based interventions ensuring pre and post work is carried out.
The post-holder will work within the stepped care model and the care programme approach, it will involve liaising with outside agencies to support patient care, whilst seeking to reduce stigma and increase access to information about recovery and resources available.
To undertake the role the post-holder is required to be registered with a professional counselling body such as BACP/UKCP.
